TP-Link TL-POE200 kit: TL-POE150S Injector & TL-POE10R Splitter
Power over Ethernet Injector with for the other end of your cable a splitter (takes power over ethernet signal and splits into plain ethernet and either 12V or 5V (switch selectable).
Supplied with ethernet cable, 2.1mm jack plug to micro USB adaptor (approx 15cm).
By using a 2.1 mm jack plug rather than 2.1mm socket, the plug to plug lead supplied by TP-Link does not need to be used, reducing the number of connections for improved reliability.

2.1mm jack to jack lead (approx 25cm) and 2.1mm jack to micro USB adaptor (approx 15cm).
